Application Checklist

Click on the application requirement to learn more about what is required and expected by the MPH Admissions Committee.

1.Start an Application through the Schools of Public Health Application Service (SOPHAS)

• All application materials must be submitted through SOPHAS.
  • Step 4 of this checklist (MSU supplemental application) cannot be completed until the SOPHAS application has been verified. 
  • SOPHAS can take up to 10 days for verification. 
  • Complete your SOPHAS application with enough time for verification before the MSU MPH application deadline.

2. Submit: Support Documents

• Required for all MPH and Graduate Certificate Applicants: 
  
  • Three letters of recommendation
  • Personal statement
  • Resume or CV
• Required for Graduate Dual Enrollment
  • Letter of support from other program
• Required for Undergraduate Dual Enrollment at MSU:
  • Undergraduate Enrollment Status Form
• Optional for All Applicants:
  • Academic statement 
  • Graduate or professional school test scores (GRE, MCAT, DAT, GMAT, etc.) 

3. Request: Official Transcripts/Test Scores

• Official transcripts
  • For MSU transcripts, be sure to follow the submission instructions here.
• Official English language proficiency test scores (e.g., TOEFL, IELTS, MSUELT, etc.)
  • For international students only

4. Start Application: MSU Supplemental Application

• Master of Public Health Program
• Graduate Certificate

5. Apply for the MSU Application Fee Waiver

• Complete the Application Fee Waiver Request Form
  • Application fee waivers are not need-based; anyone can submit a request.
  • Application fee waivers do not cover application fees required by SOPHAS.
  • Please do not pay the application fee upon submission of your application if you are applying for a fee waiver
  • Paid application fees cannot be refunded
  • Waivers will be approved after the application process is complete

6. Finish: Submit the MSU Application for Graduate Study 

• If applying for an application fee waiver:
  • Click "Finalize application and pay" and close out browser when prompted for payment
  • DO NOT PAY
